Parental Consent Affidavit for South African Government

Tour Parents,

The Department of Home Affairs for South Africa has requested that a ‘Parental Consent Affidavit‘ be completed for each student going on our tour. It is a short form that will need to be completed by you, certified by a Justice of the Peace and returned to the tour leaders. If this form could be completed and returned me (along with the 2 certified copies of student passports and 2 certified copies of your son/daughter’s unabridged birth certificate) at our upcoming parent/student meeting on Tuesday, the 27th of February it would be much appreciated.

Please remember that this particular form should be certified by a Justice of the Peace (you can find one here: http://www.courts.dotag.wa.gov.au/_apps/jps/Default.aspx). You will notice on the Parental Consent Affidavit that a certified copy of your parental passport(s) is required as part of the form as well. You can leave the school contact information and my personal contact information portion of the form blank for the moment and we can complete it together at our Tuesday meeting.

The other documents that are required (2 copies of certified Passport and 2 copies of unabridged birth certificate) can be verified by a Justice of the Peace OR anyone on this list (see link here: http://www.bdm.dotag.wa.gov.au/_files/BDM50_Certify_Documents.pdf). Having said that, you would expect that a Justice of the Peace should be able to certify all your required documents in one single sitting.
