Parental Consent Affidavit for South African Government

Tour Parents,

The Department of Home Affairs for South Africa has requested that a ‘Parental Consent Affidavit‘ be completed for each student going on our tour. It is a short form that will need to be completed by you, certified by a Justice of the Peace and returned to the tour leaders. If this form could be completed and returned me (along with the 2 certified copies of student passports and 2 certified copies of your son/daughter’s unabridged birth certificate) at our upcoming parent/student meeting on Tuesday, the 27th of February it would be much appreciated.

Please remember that this particular form should be certified by a Justice of the Peace (you can find one here: http://www.courts.dotag.wa.gov.au/_apps/jps/Default.aspx). You will notice on the Parental Consent Affidavit that a certified copy of your parental passport(s) is required as part of the form as well. You can leave the school contact information and my personal contact information portion of the form blank for the moment and we can complete it together at our Tuesday meeting.

The other documents that are required (2 copies of certified Passport and 2 copies of unabridged birth certificate) can be verified by a Justice of the Peace OR anyone on this list (see link here: http://www.bdm.dotag.wa.gov.au/_files/BDM50_Certify_Documents.pdf). Having said that, you would expect that a Justice of the Peace should be able to certify all your required documents in one single sitting.

Not Long Now!

Tour Parents and Students,

Can you believe it?! We are now have less than 50 days until we depart! As exciting as that sounds, it does mean that have little time to get things ready, including:

  • Making sure all tour payment instalments are up to date
  • Meeting fundraising requirements through the blog link to the right –>
  • Getting 2 photocopies of your unabridged birth certificates certified
  • Getting 2 photocopies of your valid passport(s) certified (and making sure you have at least 6 months remaining on it after our planned return)
  • Organising soft sided travel bags (at least 1 of them being a large duffel bag we can use for taking donations)
  • Bringing in donations to take to South Africa (soccer and netball equipment, team shirts, etc.)
  • Checking upcoming South Africa Tour events on the Calendar tab ^ .

Student and Parent Meeting Date Rescheduled to the 27th of February

Tour Students and Parents,

Due to a scheduling conflict the Student and Parent Meeting has been rescheduled for Tuesday, the 27th of February. The student only meeting will from 4-5pm in the GPLC building and the parent meeting (with students) is from 5:30pm-7:00pm, also in the GPLC building. Attendance for all tour students and at least 1 parent is mandatory, as this is our final meeting as a whole group before we depart.

My apologies for any inconvenience this change in date may have caused.

Third and Final Tour Payment

Dear Tour Students and Parents,

Just a reminder that the third and final instalment of $1050 is due on Monday, the 5th of February. This payment can be made through the link on the right side of the tour blog. –>

Secondly, please review the checklist of items listed on the the blog post from the 29th of December. The required documents will be collected at our parent/student meeting on the Tuesday, the 27th of February.

Sausage Sizzle Fundraiser – Sunday, 7th of January

Dear Tour Students and Tour Parents,

This coming Sunday, the 7th of January, a Sausage Sizzle is being hosted at Bunnings next to Whitford’s Shops by Alex S., Mitchell and Connor. Come on by and support the guys and their families in their South Africa Tour fundraising efforts. Be sure to pass the word to your friends and ask them to pass the word as well.

Let the Countdown Begin…

Tour Students and Tour Parents,

I hope you are in the midst of enjoying a wonderful Christmas and summer break as a family.

Let the countdown begin! Can you believe that its only 100 days before we climb on board the airplane to take us to Africa. With that in mind, it is time to start checking things off your list:

  • Have you already started any immunisation program you may wish to acquire through your GP or travel nurse?
  • Have you organised a large duffel bag (or 2)?
  • If you are also an Australian citizen AND a South African citizen, do you have a valid passports for both countries (both passports are required for dual citizens)?
  • Have you ensured that there is at least 6 months validity remaining on your passport(s) for AFTER we return from Africa?
  • Have you prepared 2 certified copies of your passport(s) and your unabridged birth certificate?
  • Have you reached the required individual fundraising amount of $700? If so, keep going! 🙂
  • Have you placed a reminder on your personal calendar for the next student and parent meeting (Tuesday, 13th of February)? The student meeting is from 4-5pm in the GPLC and the parent meeting (with students) is from 5:30pm-7:30pm in the GPLC. Attendance for all tour students and at least 1 parent is mandatory, as this is our final meeting as a whole group before we depart.
